Workplace survey with Clixsense?

Hi- Has anybody done a work survey with Clixsense? I just did one that I backed out of right before I completed it, because they asked for the name of my supervisor, and his email address, so they could also send the survey to him, and they would tell him who supplied the information. I just go no I am not doing that. How many people are going to supply that information? They say that they won't disclose what I said about my job and supervisor to my supervisor, but this is just unreal to expect anybody to allow Clixsense to send a survey to their supervisor, and then tell the supervisor who gave them the information.
