I have cash out twice with them since 12/1/15. Each time I received the check within 4 to 6 weeks. I think some of it is base on time of the month. They may only write checks on the 1st or 15th for example after being approved which may add another 2 weeks of waiting.

It was either last year or the year before that, I was blocked twice for the same reason. Thankfully both times they allowed me to come back. A lot of people have had this problem with 'proxy severs' as being the reason that they're banned from taking surveys.
